Pterygium surgery reimagined.
Hi, I’m Dr. Rylan Hayes, specialist cataract, refractive, and pterygium surgery right here on the Sunshine Coast. Traditionally, pterygium surgery was focused on one thing: removing the growth. Not much thought was given to how the eye looked afterwards, or how uncomfortable the recovery was. That’s changed significantly over time — and I’ve taken that change further with my Signature no-stitch Pterygium Surgery technique. My approach puts a triple focus on cosmetic outcome, patient experience, and functional excellence. I use a premium tissue adhesive called Tisseel — made from donated blood products — instead of traditional stitches. Pterygium Surgery — Reimagined for decades, pterygium surgery was a purely functional procedure. The goal was simple: remove the abnormal tissue growing across the surface of the eye, and reduce the chance of it coming back. The patient’s appearance after surgery, and the discomfort they experienced in recovery, were largely secondary considerations. That approach no longer reflects what patients expect — or what modern surgical technique can deliver.

The problem with traditional pterygium surgery.

Conventional pterygium removal using sutures (stitches) to secure the conjunctival graft is effective, but it comes with a significant downside: pain. The sutures used to hold the graft in place sit on the surface of the eye — one of the most sensitive structures in the human body. Many patients who have undergone traditional suture-based pterygium surgery describe weeks of persistent, significant discomfort. Some recall the experience vividly years later.

Beyond comfort, traditional techniques often prioritised tissue removal over appearance. The result could leave the eye looking irregular, red, or scarred — not the natural, clear eye a patient was hoping for.

Dr Hayes has developed a premium pterygium surgery technique that reimagines the procedure from the ground up. The three guiding principles are: cosmetic excellence, patient comfort, and low recurrence.

Instead of sutures, Dr Hayes uses Tisseel — a premium fibrin tissue adhesive derived from donated human blood products. Tisseel securely bonds the conjunctival autograft (a small piece of the patient’s own healthy tissue taken from under the upper eyelid) to the surgical site. This approach:

• Eliminates suture-related discomfort — most patients report only mild discomfort for a day or two, with many requiring nothing more than paracetamol
• Produces a more natural cosmetic result — no suture marks, no irregular tissue edges
• Maintains low recurrence rates — the autograft technique is the gold standard for preventing regrowth

Patients are still provided with prescription pain relief in rare cases it is needed for breakthrough discomfort — but the experience of no-stitch pterygium surgery is a significant step forward from what previous generations of patients endured.

Why cosmetic outcome matters.

The eye is one of the most visible features of the face. A pterygium — the reddish-pink fibrovascular growth that encroaches onto the cornea — is visible to others and distressing for many patients.
